Athletic Sports Packet Information 

2010-2011 School Year

Dear Parent and Athlete,

Welcome to the Athletic Department at Foothill High School. All athletic programs at Foothill High School stress sportsmanship and fair play.  Participation in the Athletic Program depends upon completion of ALL mandatory forms, ability in the sport, and compliance with all other rules of eligibility. In addition, each student participating in athletics pays a contribution which helps maintain the operation of the Foothill Athletic programs. It is recommended the sports packets be turned in to the Main Office two weeks prior to the first day of practice. 

The 2010-2011 athletic packet forms are now on our website for you to read and download.
 
Please use the guideline and instructions below to assist you in completing the Athletic Packet. Return ALL mandatory forms with the necessary signatures to the Main Office. DO NOT GIVE THE FORMS TO THE COACH.  There are NO exceptions. 

  1. Read the Student Handbook for Interscholastic Athletics and Co-Curricular Activities.

  2. Read the Non-Use Steroid Agreement And Athletic Participation Agreement.  

  3. Read the P.U.S.D. Athletics/Activities Code of Conduct. 

  4. Submit a current Physical Exam Form signed and dated by a medical doctor (M.D.).
    Every school year a physical examination or statement by a medical doctor is required certifying that the student is physically fit to participate in athletics. The physical exam or statement cannot expire during the sport season. Please note: A chiropractor's assessment is no longer valid. For your convenience, sports physicals will be offered at Pleasanton Urgent Care, located at 3128 Santa Rita Road, Pleasanton, (925-462-9300). In addition, Valley Care Sports Medicine will be offering a sports physical clinic. For information, please call (925-373-4019) 

  5. Emergency Information Form – Complete in full – Sign and date. Submit to office.

  6. Insurance Information – Complete in full - Check the appropriate box, sign and date. Submit to office.

    The school does not provide medical insurance coverage for school accidents. However, voluntary student insurance is available through UnitedHealthcare StudentResources. If you choose to purchase this insurance, please come into the Main Office and a UnitedHealthcare brochure can be given to you. Or for fast, easy enrollment go online at www.k12StudentInsurance.com.   

  7. Informed Consent Agreement Form - Complete in full - Sign and Date. Submit to Office.

  8. Enclose the contribution amount assigned to the particular sport made payable to Foothill High School. Once the team roster has been submitted to the Athletic Director, the only APPROVED refund will be for injury/medical reasons. If the player chooses not to participate in the sport there will be NO REFUND.

  9. If you participate in a 2nd and/or 3rd sport, only 2 items are required to be submitted:
    • Emergency Information Form–Complete in full, sign and date.  Submit office.
    • Contribution amount assigned to the particular sport made payable to Foothill High School. 
    • If your insurance coverage has changed, resubmit a new Insurance Information Form.

  10. Athletic Packet Checklist/Signature Form – Complete in full - Sign and date.  Submit to office.

  11. Attend the mandatory sports meeting. The meetings will be held in the Large Gym at 7pm. The following is the EBAL 2010-2011 season scheduled openings and Foothill’s corresponding evening meetings:
                          Parent/Athlete Meeting              Practice/Tryouts Begins
Fall Sports:       Monday, August 9, 2010              Monday, August 16, 2010
Winter Sports:  Monday, October 25, 2010           Monday, November 8, 2010
Spring Sports:  Monday, January 10, 2011           Monday, February 7, 2011
